TREATMENTS

It is never too early to prevent hair loss. There are several risk factors that can be identified and with the right therapy it is possible to stop baldness.

With hair transplant surgery you can obtain up to 2000 follicles, each containing up to 4 hairs, for each day of surgery from 3000 €

Surgery is virtually painless and is usually performed one day.

BALDNESS

The scientific term for male pattern baldness is Androgenetic Alopecia. Alopecia means hair loss. Andro comes from the androgenic hormone called testosterone and genetics because it is linked to heredity.

That is, in men, about 95% of baldness situations are directly related to heredity and the other 5% are linked to traumatic situations, stress and infections. However, it is not testosterone that acts directly on the hair root receptors, but Dihydrotestosterone (DHT).

Most men accept this loss without major psycho-emotional damage, but others will accept in a very negative way the premature loss of their hair, and may even be victims of emotional and professional problems, worsening their quality of life.

To give you an idea, at 30 years of age approximately 30% of men have Androgenetic Alopecia. At 50 years old, approximately 50% of men have Alopecia Androgenetica. It is in the white race that there is more Alopecia. Baldness rarely manifests in adolescence. It is common to appear after adolescence and to manifest more aggressively between 20 and 40 years of age. After their installation, some men are completely bald in half a dozen years, but most will take 15 to 25 years to reach a high state of baldness. After 55 years of age, the baldness process rarely begins, since hormone levels are low.

PREVENTION

There have been several treatments proposed for hair loss since the beginning of human history. From the use of herbs, oils, ashes, electric shocks, massages, lasers, wigs, etc. However, the results were never satisfactory. The basis of medical treatment is the reduction of DHT and that of androgen receptors against the action of DHT. Usually the benefits obtained are in the reduction of the speed of installation of the baldness, and they only appear 6 to 8 months after the continuous use of medication. There is still no short-term medical treatment. Therefore, in order to have results, the patient will always have to undergo continuous treatment between 20 and 50 years of age, eventually suffering the consequences of continuous medication. Although there are multiple drugs on the market, Dr. Dinis Cruz suggests only the following, which have some effectiveness, depending on the indication:

 

- Finasteride
It is a potent inhibitor of the enzyme 5 alpha reductase type 2, which was approved by the FDA for the treatment of Alopecia Androgenetica in 1997 under the trade name Propécia (R).
In 51% there was stabilization without an increase in baldness.
Finasteride is a good medicine for individuals in the early stages of androgenetic alopecia, however if there is already alopecia in any area, it will be very difficult to solve the problem with this medication alone.

 

- Minoxidil 5%
After years of experience, it was agreed that less than 15% of men treated with 2% minoxidil had slight benefits with increased scalp hair, with no apparent improvement in hair aesthetics.
Currently, 5% minoxidil is used but its use alone is not believed to promote major benefits in androgenetic alopecia.

 

- Plasma Microinjection Rich in Growth Factors (L-PRF)

It is a technique that consists in the collection and centrifugation of the patient's blood to obtain a serum concentrated in the individual's own cells, so there is no risk of rejection, rich in growth factors, which will be injected into the receiving area helping to induce and strengthen the formation of hair follicles. It alone does not resolve a large alopecia, but works as a potent co-adjuvant to hair transplantation.

FUE TECHNIQUE

The FUE technique (follicular unit extraction) is used in hair transplantation, as well as other hairs on the body, it consists of using punch preferably up to 1mm in diameter and removing follicular units from the donor region and placing one by one in the recipient region.

Of course, the use of punch is not new, it is old, since Dr. Tamura in 1944 already used them to put pubic hair on women. However the big difference is in the size of the punch, currently using a punch for each follicular unit.


Using the FUE technique, the work of having to separate the follicular units is avoided, as it is not necessary to make cuts, so this technique is less invasive.
This technique requires a lot of experience and must be performed manually, always respecting the shape and position of the follicular units to be removed.

- Ideal for young patients who like to shave their heads;
- Less invasive, without cuts and leaving no scars.

HAIR TRANSPLANT SURGERY

Before starting the surgery, the head is washed and then the appropriate measurements and markings are made.

Subsequently, the patient is medically evaluated, the vital signs are checked, everything is fine, the patient is slightly sedated. Then anesthesia is performed, which is local and trunk.


For us it is extremely important and fundamental that our patient remains calm in contact with the doctor and does not feel pain during the entire surgical period, for this reason Dr. Cruz Dinis is very special, to prick or cut only when he is sure the patient can no longer have pain. For that, he uses cream with local anesthesia and a special syringe called Dermojet, which avoids any type of pain when introducing anesthesia.


The surgical procedure varies according to the technique used:
In the case of Strip Harvesting, the surgery begins with the removal of the scalp strip from the donor area, the size of which will depend on the number of follicular units to be transplanted. After this intervention and before the surgical wound is closed, the upper edge of the surgical wound is trichophytized, thus allowing that after healing it is possible to grow hair over the entire surgical scar.
After trichophytosis, the suture is performed. The end result is a thin, delicate scar that is barely visible and is completely covered by hair.


The follicular units are then separated by a specialized team in which the units are separated according to the number of hairs present in each one. This separation is extremely careful so that there is no damage to the follicular units, preserving their anatomy and blood supply, thus ensuring the integration of all transplanted units.
The placement of follicular units is carried out with special new generation implants (Japanese and Korean), allowing results with high capillary density, without leaving any kind of mark.


In the FUE technique, the surgery begins by shaving the hair from the donor region.
After the anesthetized region, special titanium punsh (from the USA), with a maximum diameter of 1 mm, are used in order to extract a few hundred follicular units from the donor region, one by one, and later placed in the recipient region, always obeying to the inclination and angulation of natural hair. The introduction of follicular units is similar to that of the Strip Harvesting technique.


Regarding the recipient region, and especially in situations of frontal baldness, it is extremely important to pay attention to the anterior frontal line of the head. In these cases, Dr Cruz Dinis uses a method that he called the Viroj line, in honor of the creator of this method, Dr. Viroj Vong.

For years, Dr. Viroj photographed hundreds of monks in order to understand how the hair is distributed in the frontal region of the head.
He then classified the frontal lines into 3 different types. From here he started to create molds, based on the hundreds of front lines he had studied. Using these molds on his patients he obtained very natural looking frontal lines, thus obtaining excellent results.

At the end of the surgery and after the placement of all follicular units, the dressing of the donor region is made and a specific substance is placed over the recipient region in order to protect the hair transplant.

On average this surgery takes between 5 to 10 hours, depending on the amount of follicular units that are placed.


At the end of the surgery, the patient before being discharged is reassessed by Dr. Cruz Dinis, then takes a snack, returning the next day to remove the dressing, head washing and post-operative counseling.

PRE-SURGERY

Before surgery, you should take care and during the preoperative consultation, share with Dr. Cruz Dinis all the important information for your surgery:

 

1. Medicines: You must inform if you are taking any medication and which. If you take aspirin or the like, you should stop taking it 2 weeks before surgery. If you take Vitamin E you should also stop taking it 2 weeks before.

 

2. Alcoholic beverages: You should not consume alcoholic beverages at least 48 hours before surgery.

 

3. Tobacco: You must not smoke at least 24 hours before surgery.

 

4. In the Strip / FUT technique, do not cut your hair. In the technique of FUE shaving or cutting very short.

 

5. On the night before surgery try to relax, if necessary you can take a tranquilizer, as it is important to sleep well.

 

6. On the day of surgery take a normal shower and don't worry about washing your head, because before the surgery we will wash your head with an antiseptic shampoo.
Come dressed in comfortable clothes and preferably with a button-down shirt, do not wear sweaters that you need to wear over your head.
Have a light breakfast, avoid tea or coffee.
Bring a cap or hat that is not tight, as you will need it to protect your head from sunlight, pollution and also people's eyes.

 

7. Don't be late and be at the Clinic at the appointed time.

POST-SURGERY

After finishing the surgery and when arriving home, the patient should take some precautions that include the application of ice on the forehead for 15 minutes every hour, to avoid swelling.

It is also important for the patient to be accompanied by someone, at the end of the surgery and preferably not to drive, because sometimes sedation, despite being mild, in some patients makes them quite sleepy.

 

The next day the patient returns to the hair transplant center and the doctor will wash and teach you how to wash your head.

This procedure performed by Dr. Cruz Dinis will give the patient more security to continue washing at home, without fear, which should be performed preferably 2 to 3 times a day.

Before the patient leaves the Hair Transplant Center he is informed by Dr. Cruz Dinis about what medication to take and the new post-operative care.

Following these guidelines, the post-operative period of 2 to 3 days is sufficient for the full recovery of their routine activities.

1. What is a hair transplant?

2. Does it work?
Yes, the hair that is transplanted lasts a lifetime and will be growing naturally, at 8 months and within a year they look great.


3. Will I get a head full of hair as a result?
